New Patient Registration  

We are pleased to welcome new patients to the practice list. In order to register, please enquire at reception where you will be given a purple and white registration form and a New Patient Questionnaire to complete.

Alternatively, these forms can be downloaded from the above “Registering Online” tab.

When completed, these forms should be returned to our reception team any time between 9.00am - 6.00pm.

We will also need to see proof of your identity and you will be asked to provide the following:

• ONE type of photo identity, such as your passport or driving licence
• TWO types of proof of address, such as a recent utility bill (gas, electricity, water or phone bill, but not a mobile phone bill) or council tax bill, rental contract or recent bank statement.

You will then be asked to make an appointment to see a member of our practice nursing team for a new patient check.

Changes to Personal Details

If you change your name, address or telephone number please let us know as quickly as possible so that our records can be updated.

You can change your details by telephoning the reception team on 01386 761111 or by calling into the surgery to fill in a change of details form.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. In order to complete the registration we require you to come into surgery and sign the documents and bring in your ID.

Please ensure that both of the above forms are fully completed before handing into our Reception Team, Failure to complete both forms will delay your registration.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively, you may print off a registration form &  New patient Questionaire , fill it out and bring it in with you, plus your ID, on your first visit to the practice.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
