New Patient Registration
We are pleased to welcome new patients to the practice list. In order to register, please enquire at reception where you will be given a purple and white registration form and a New Patient Questionnaire to complete.
Alternatively, these forms can be downloaded from the above “Registering Online” tab.
When completed, these forms should be returned to our reception team any time between 9.00am - 6.00pm.
We will also need to see proof of your identity and you will be asked to provide the following:
- ONE type of photo identity, such as your passport or driving licence
- TWO types of proof of address, such as a recent utility bill (gas, electricity, water or phone bill, but not a mobile phone bill) or council tax bill, rental contract or recent bank statement.
You will then be asked to make an appointment to see a member of our practice nursing team for a new patient check.

Changes to Personal Details
If you change your name, address or telephone number please let us know as quickly as possible so that our records can be updated.
You can change your details by telephoning the reception team on 01386 761111 or by calling into the surgery to fill in a change of details form.
